Lakeland Care Inc. Announces New Private Pay Care Management Service

Care Plus is designed to coordinate care needed to support an individual’s highest level of independence possible

Lakeland Care, Inc., an organization focused on building better lives through programs that care for and inspire our communities, is proud to announce Care Plus, a private-pay care management option designed to help people access the services they need to live at the highest level of independence possible. Care Plus does not have a financial-need requirement, expanding the availability of Lakeland Care’s care management services.

“Often family caretakers are managing a household of their own, in the workforce or balancing other commitments,” said Sara Muhlbauer, CEO of Lakeland Care, Inc. “We know managing a loved one’s care can be overwhelming and that’s where Care Plus can help. Care Plus will identify care needs and coordinate essential services to help individuals live their most independent life.”

Care Plus will assist clients in identifying care and service options that meet their needs and personal goals and will coordinate the individual’s selected services, including providers. Clients will privately pay for their chosen services and each client will also be assigned their own personal care manager, which may be a licensed Registered Nurse or a social service professional.

This program aligns with Lakeland Care’s long history of care management as part of the State of Wisconsin’s Medicaid programs. Care Plus is available to those who do not qualify for Family Care.
